I'm glad there's interest in this...I'll start working on putting one together...I'm going to make it as detailed as possible, and maybe we can learn something. I think the best way to do it will be that I'll create a document and upload it to the files section, then whoever wants to fill it out can download it and then I guess email it to me? I'm thinking that if we could get enough people to contribute, maybe we could identify some trends that would help us all gain a better understand of this condition and how to cure it (I know, I know, there's no "cure," but I think eventually there will be...).
