Oral antifungal bad for papulas/rosacea?
Hi, a question from Holland, ones more.
I wonder if anybody can tell me if an oral antifungal like sporanox
might cause more red papulas and spots.. I´ve got rosacea, acne and
seb derm, and the last seems to get worse lately. Because of the
rosacea and superirritated skin I can´t use topical creams to combat
it and as my cheeks are getting yellow and flaky and more inflamed
and ´spotty´ lately, I´m thinking of using the Sporanox my derm gave
me. But I don´t want to use it ´unprepared´, as with the doxycycline
I used a while ago for three months and which gave me far worse seb
derm (I didn´t realize that in front..). So: could it be possible
that a course of antifungal medicine gives me more papulas, redness
and rosaceaproblems?
I´m on a death spot at the moment, and something has to be done,
after three months doing nothing and seeing (and feeling) things
getting worse and worse.
Can anybody please inform me about this? I wonder as well why I get
so many red papulas and spots in these yellow, rough areas, all over
my cheeks. Can anybody explain it to me? Is it from the rosacea or
from the seb derm?

Hi Natalie,
Did you ask your dermatologist to get you on a course of ACCUTANE?
I know DR Nase recommends this as something that will reverse the
whole desease process.
He says an antihistamine tablet every day is good, like zyrtec 10 mg.
And then accutane.

Here is what Nase says about low dose accutane on his page.
–
2. Isotretinoin (Accutane) - this is a vitamin A analog that has been
used with much success to treat severe nodulocystic acne, disorders
of keratinization and various other dermatological diseases.
Isotretinoin works by inhibiting sebaceous gland function and
physically shrinking these problematic glands. It also has potent
anti-inflammatory properties, which make isotretinoin a good option
for the treatment of resistant rosacea patients. It achieves long-
term benefits and remission of papules, pustules, rhinophyma and a
decrease in facial burning. Another benefit is that it allows
patients to discontinue the use of acne products resulting in less
facial irritation. Some do experience a relapse, but the symptoms are
usually much milder. Many physicians find that 1 or 2 courses of low-
dose isotretinoin may significantly decrease the severity of the
entire disease process. Side effects include dry lips and skin, a
transient elevation in cholesterol and triglyceride levels, mild
headaches and dry eyes. One serious side effect is that it can cause
severe deformities to the unborn fetus. Thus its use is
contraindicated for women who are trying to get pregnant or who are
not using birth control while sexually active. With this said, the
use of low-dose isotretinoin can be a godsend to many rosacea
sufferers.

Hi,
I dont think you should bother with trying tea tree or evening
primrose oil. I was a bit surprised that the aloe vera worked for me,
but I dont think there are many things that work too well for acne
and seb derm. Of course, trying to get the right fats in your diet
might on a longer term help some, but I would not hope for any quick
help from that. I do think getting omega 3 and other fat is good for
you, and it wont make your skin worse, but maybe not much better.
But again, since you said you have both acne and seb derm, I would
say you should try a course of acutane, either low dose, as Dr Nase
talks about, or regular. Ask your dermatologist.
I did use oral antibiotics, not oral antifungals. I dont know
anything about them. What I said was that oral antibiotics
(tetracycline) did not work very good for my acne, and I think it
made my seb derm worse.
>and since the tetracyclines made my seb derm so much worse, I
>wouldn´t be surprised if it worked the other way round as well.
Yes, I had a problem with tetracycline as well. My opinion about
antibiotics for acne, is that it is a total waste of time. If I knew
about it then, I would tell my dermatologist to quit being a jerk and
put me on accutane. I dont care that some people say it will kil you,
cause it wont. The heartbreak of having skin problems is much worse
than the side effects of accutane.
>I feel very frustrated, scared and out of control often, especially
>when my skin deteriorates a lot in a short amount of time.
I know what you are saying. It is a bitch. The best thing is to try
to find the best treatment, and do what you can to get that
treatment.
>I try to use Vaseline as a basis for the topical nizoral, because I
>know that on certain spots I can tolerate it.
1. Noritate cream — Have the compounding pharmacy
compound 2% ketoconozole into the noritate. Make
sure they pulverize the ketoconozole into
microfine particles. With this special cream you
have a proven anti-inflammatory cream in a non-
irritating base with a powerful anti-seb derm medicine.
I also recommend that you add a drop of safflower
or chamomile oil to the mix.
2. Noritate cream — Have the compounding pharmacy
compound 1% zinc pyrithione into the noritate.
3. Noritate cream — Have the compouding pharmacy
compound 2% precipitated sulfur into the cream.
